Tanzania is a hub of commerce and migration, and the country has made great strides in reducing HIV incidence rates, as evidenced by the Tanzania HIV Impact Survey (THIS) implemented by the Ministry of Health and ICAP in 2017. However, key and hard-to-reach populations are still at increased risk. ICAP partners with the Ministry of Health to improve HIV prevention, care, and treatment for these populations and to explore innovative ways of offering services and linking individuals to care. ICAP also supports the Ministry in addressing the global health security agenda for emerging threats such as COVID-19 and Ebola Virus Disease on disease surveillance and laboratory system strengthening for the Integrated Diseases Surveillance Response (IDSR) system.

Position Summary:
The Case Management Officer (CMO) ensures client-centered services are provided to all people living with HIV (PLHIV) with fidelity. The CMO is responsible for facilitating the continuity of services and establishing networks for supportive services such as psychosocial counseling, legal services, economic strengthening, and PLHIV support groups. S/he will ensure client feedback is routinely collected and analyzed and information is used to make timely decisions for the benefit of the clients. The CMO will work with other ICAP staff to improve patient literacy, medication adherence, and care retention including differentiated service delivery (DSD) approaches. S/he will work with the facility providers to ensure accurate recording and regularly update clients’ demographics and treatment supporters’ information for easy tracking and follow-up.
Roles and Responsibilities:
- Assist patients, families, and caregivers in navigating the comprehensive care and treatment journey by collaborating with the facility providers.
- Assist facility staff and knowledgeable clients in achieving the highest possible degree of wellness for PLHIVs by offering pertinent patient education, prompt referrals, and client-centered services.
- Along the continuum of care, collaborate with the CHMT to identify relevant service providers, stakeholders, and providers, and create referral systems that allow clients to access services.
- Encourage accurate recording and frequent updates of patient data in facilities to facilitate simple follow-up.
- Assist the M&E and CQI teams in making sure that ART retention services are properly recorded and reported.
- Encourage the daily implementation of efficient tracking and retention services, such as DSD techniques to guarantee ART clients stick to ART and clinic schedules, in order to address program, donor, and national priorities.
- Assist medical professionals in promptly identifying neglected appointments and spearhead initiatives to get patients back into the fold
- Make sure that CTC/PMTCT/TBHIV clients are properly tracked at the facilities, and minimize treatment disruptions.
- Make that every facility in the relevant council has access to and is using the common national tools for scheduling appointments, tracking down clients, creating map queries, and assisting with the necessary paperwork and reporting.
- Compile and evaluate the council’s daily, weekly, monthly, and quarterly monitoring reports, and assist facilities in using data to make decisions.
- Organize and lead monthly sessions where participants can discuss obstacles and solutions for enhancing tracking activities.
- Help sites comprehend goals, match their tactics and endeavors with them, and achieve their predetermined performance objectives
- Carry out any additional tasks that the supervisor assigns you.
Qualifications, Knowledge, and Skills:

- Required Education: Degree or Diploma in Social Work, Medicine, Nursing, Public Health or a related field
- Required Technical Experience: Minimum 3 years of experience in the HIV field including demonstrated experience with client-centered case management of PLHIV o Preferred: Experience working with PEPFAR-funded HIV programs
- Experience in using CQI techniques to address public health challenges
- Experience in writing reports, best practices, and lessons learned
- Excellent command of Swahili and English languages in written and oral communication
- Experience in basic computer applications such as MS Word, Excel, PowerPoint, and the internet
- Ability to work under pressure and tight deadlines
